Job Description

Barker Ross Health & Social Care are recruiting for experienced Support Workers to provide high quality support to adults with complex support needs. Candidates are required to enable clients to live their life, their choice and their way. They will do this by providing clients with support in a range of areas, that include health, emotional and social support. We are recruiting for candidates who will engage with clients in order to understand them on an individual basis, gaining an insight into their unique needs, wishes and aspirations, and delivering person-centred care according to individualised support plans.

Senior Care and Support Worker - Mental Health (Chelmsford), Permanenet, 37.5 hrs per week, £17,839pa

We are currently seeking a Senior Care & Support Worker to join a supported living service; supporting the needs of Adults aged between 18-65 with mental health challenges in Maldon, Essex. This is a referral service working on a 24-hour basis providing accommodation through six individual self-contained flats.

You will manage cases and supporting the general well-being of residents on a daily basis, you will do so by working alongside our customers to develop, review and update of care and support plans and risk assessments. You will encourage and support your assigned caseload to establish and maintain positive relationships with their family and friends and engage in activities which help them to achieve their personal goals.

Main accountabilities:

-Work with customers to develop, review and update of care and support plans and risk assessments in line with policy.
-Deliver care and support in line with care and support plans and risk assessments.
-Encourage and support customers to establish and maintain positive relationships with their family and friends and engage in activities which help them to achieve their personal goals.
-Provide support and practical assistance to customers around benefits, budgeting, paying bills and accessing appropriate services, including some housing management.
-Provide social and emotional support to customers in line with policies & procedures and best practice.
-Promote self advocacy and act as the customers` advocate where appropriate.
-Be aware of customers` wellbeing, safety and state of health, reporting any concerns.
-Ensure all necessary records are accurate and up to date.

About you:

-Minimum of 6 months mental health experience
-Driving license and access to a vehicle
-Reliable, responsible, non-judgmental
-Ability to work flexibly to meet customer and service requirements across a geographical area
-Availability for various shift patterns between 8:00am -10pm plus a minimum of 2 sleep ins per week.
-On-call service where appropriate
-Ability to work using own initiative
