Job Description

Qualified Social Worker - Adults - Gainsborough

An opportunity is available within an Adults & Older Persons Team In Gainsborough. You will help to empower vulnerable adults to make choices and to maintain their own autonomy wherever possible.

Experience in adult social care in the community. Experience and knowledge of CHC, DST's hospital discharge, MCA, best interest meetings, assessments, care planning, safeguarding in the community, full knowledge of adult care legislation and the care act

Duties include:

  • Experience and knowledge of CHC, DST's hospital discharge.
  • Undertaking a range of assessments using the Mental Health Act and undertaking Mental Capacity Assessments.
  • Safeguarding clients
  • Arranging appropriate care in line family and cares guidance
  • Working with partnership agencies
  • Implementing Direct Payments
  • Assessing Deprivation of Liberty
  • Reviewing cases
  • Applying for funding and presenting cases to panel.
  • Preventing re-admission to hospital where possible

Essential Requirement:

You must hold a social work qualification (BA / MA Social Work / DipSW or equivalent), and hold current HCPC registration.

You must have experience of working in a similar role.
