Clinical Community Nursing Specialist - Greater London

Job Description

Our client is one of the largest nursing and care providers. They are currently seeking a community Nurse.

Position Available: Clinical Community Nursing Specialist - Greater London and Kent, Permanent, 37.5 hrs per week, £38k

Role Purpose:

Practice within the Legal & Ethical framework as established by Nursing & Midwifery Council (NMC) and National Legislation to ensure clients interests and well-being are met.

To take responsibility for the clinical care of each client within your caseload, reporting to the Nurse Manager.

To implement, assess and evaluate a care plan for each client.

To make regular visits to each Client within your caseload to meet the clinical needs and training requirements for each package

To take responsibility for the training and assessment of competencies for the personal assistants working within each team.

To liaise with the multi-disciplinary team as required for each client in your care.

To ensure best practice is implemented and the highest standards of individualised care are met.

Responsibility for maintaining client records within agreed standards.

Provide telephone support to Clients and Personal Assistants as part of a 24 hour on-call rota

Key responsibilities


Communicate effectively with clients and family members.

Communicate effectively with care managers, personal assistants and the wider multi-disciplinary team to ensure client safety is maintained

Attend meetings as required with other agencies, hospitals, District Nurses, GPs to ensure continuity of patient care

Communicate effectively with the Company and Management Team, highlighting any problems or concerns at the earliest opportunity.

Complete a weekly/monthly written report for each client.

Personal and Company Development

Take responsibility for personal continuing professional development in order to enhance knowledge, skills and values needed for safe and effective practice and to meet PREP requirements

Work with the company on any new training requirements for yourself and other staff members.

To recognise and understand the roles and responsibilities of individuals working in the company.

Health, Safety and Security

Identify the risks involved in care and work activities and undertake them in a way that manages the risks.

Use appropriate infection control procedures ensuring that clients homes are clean, safe and free from hazards and to report any potential risks.

Know the general principles of First Aid and resuscitation and to be able to undertake initial actions as appropriate.

Enable mobility, including Moving and Handling techniques and appropriate use of equipment.

Know the Health and Safety policies and procedures within the company and clients own homes, including fire procedures, maintaining documentation, monitoring and maintenance of equipment.

To comply with relevant medication policies and legislation relating to medication administration

Clinical Skills - Health and Well-being

Implement, assess and evaluate a Care Plan for each client.

Select and implement evidence based nursing interventions when appropriate to meet the individual needs of clients using National Guidance

Ensure personal assistants and clients are able to undertake record and follow clinical guidelines for the tasks for which they have received appropriate training.

Ensure personal assistants are fully trained and have their competency assessed for all care procedures they are required to undertake for their Client.

Information Processes and Reporting

Complete a weekly report on the clinical care, health and well being of the client and the training needs of the personal assistants

Maintain confidentiality of information relating to clients, relatives, staff and the company at all times in line with the data protection act and NMC code of conduct

Ensure that reporting is done in an accurate and timely fashion in line with company policies and procedures.

Management Key Responsibilities

Effectively manage own work load, prioritising clinical need and training within each package of care

To ensure that company policies and procedures are understood and followed by PAs.

To provide support and guidance to PAs in the completion of their duties and to coach and develop them in delivering a high quality service.

Attend team meetings within the packages and management meetings at head office as required

Work closely with the Care Manager to ensure the smooth running of each package.

Experience required:

Level 1 NMC registered (Adult) Nurse

Specialist field of practice - Intensive Care Nursing, High Dependency Nursing, Spinal Injuries or Community home ventilation experience

Excellent interpersonal and communication skills

Excellent clinical care skills

Ability to work as part of a team and on own initiative

Management experience

Ability to prioritise case load effectively

Problem solving & crisis management

Quality management

Accurate reporting skills

Able to promote and represent the Company in a positive and professional light

Able to convey information clearly, follow organisational policies & procedures and produce appropriate written communication

Willingness to work flexibly to meet the needs of the business
